Office Policies


 

Children Present At The Appointment Time

Due to OSHA regulations, and for the safety of your children, no child will be allowed in the back office. Please make necessary arrangements for your appointment. We are enforcing this policy effective immediately. No child under 12 years of age will be allowed to wait in the waiting room alone. Again, this is for the safety of your children.

 

Missed Appointments

Our goal is to provide quality care to all of our patients. When an appointment is scheduled, a block of time in the doctor’s schedule has been allotted to you, the patient, to allow adequate time to address all of your concerns and provide medical treatment.

We reserve the right to charge a $25.00 fee for all missed appointments and appointments that have not been canceled 24 hours prior to the scheduled appointment time.

 

Late Appointments

We request all patients to arrive on time for their appointment. Please call the office if you think you may be running late. We understand there can be unexpected delays and we will try to accommodate.

However, if you are more than 15 minutes late we reserve the right to reschedule your appointment. This may be necessary to avoid inconvenience to other patients.

 

Patient Responsibility

You are financially responsible for all services provided if:

  • You are determined not to be eligible for insurance coverage
  • The services are not covered under our benefits plan
  • A referral and/or authorization is needed (for office visits and/or procedures) and not obtained
  • You are seeking services “out of network” with a non-contracting provider

Co-payment is expected at the time of service.

 

Disability Forms/Letters

There is a $35.00 fee for all forms/letters needing to be completed by the physician/office staff. These forms include, but are not limited to, Disability forms, Family Leave forms, etc. There is also a $35.00 fee for those of you requesting a medical letter for Visas for a family member.

 

Medical Records Release

There is a $35.00 fee for copies and/or transfer of any and all medical records accompanied by a written authorization from the patient or the parent of the minor patient. We do not fax any records for any reason. Records from other providers need to be obtained directly from that provider.

Insurance

Most insurances are accepted. The percentage of coverage does vary with policies. To avoid confusion, it is best to review your personal policy or consult your insurance carrier. Our staff can answer any questions you may have, concerning insurance.
